How do I notify amaysim?

Who can notify amaysim?
A representative who can act on behalf of the deceased account holder may submit a request. This can include:
Executor, administrator or trustee of the estate
Next of kin, such as an immediate family member or guardian over 18
Civil and Administrative Tribunal
Solicitor or lawyer administering the will
Authorised representative listed on the amaysim account
What documents do I need?
Please send the completed and signed form with the documents that apply to your situation.
To confirm your authority to act
Provide one of the following:
A statutory declaration confirming your authority to act
A letter confirming that you are the executor, administrator or trustee of the estate
A letter from the solicitor or lawyer administering the will
To confirm your identity
Provide one current, unexpired identity document listed in Part A of the form:
Australian driver licence
Passport
Green Medicare card
If the transfer is going to a new account holder, that person will also need to provide one current, unexpired identity document listed in Part C of the form.
After the form and supporting documents are received, amaysim will contact the representative.
To confirm the account holder’s death
Provide one of the following:
Death certificate
Death notice
Doctor’s medical certificate
Grant of Probate
Letters of Administration
Funeral bill
Link to a funeral notice or obituary
Why does amaysim require these documents?
We need to verify that the person requesting a transfer or cancellation has the appropriate authority to act.
Frequently asked questions
How do I transfer a service?
Complete the transfer details in Part C of the Transfer or close an amaysim account due to loss of a loved one form. If services are being transferred to different people, submit a separate form signed by each new account holder. Each new account holder must complete the required identity verification before the transfer can be processed.
Will the existing plan and discounts transfer?
The service will move to the new account holder once the transfer is complete. Any plan, inclusions, discounts or promotions will be forfeited when transferred to the new owner.
Who is responsible for charges after a transfer?
The new account holder is responsible for charges incurred from the date the transfer is completed. Any existing direct debit arrangement will be cancelled, and the new account holder can set up their own payment method through My amaysim or the amaysim app.
What happens to any credit remaining on the current service?
Any remaining credit on the current account or service will be forfeited and won’t be transferred to the new owner. If possible, it’s worth using any remaining credit before the transfer is completed.
How do I cancel a service?
Complete the cancellation details in the form and include the supporting documents that apply to your situation. Once we’ve reviewed the request and confirmed we have all the information required to do so, our specialised team will cancel the service and send confirmation when it has been completed.
What happens to billing and direct debit when a service is cancelled?
Once the cancellation is processed, we’ll stop further billing for the cancelled service and cancel any existing direct debit arrangements linked to it.
What happens to voicemail after I cancel a service?
A voicemail greeting on a cancelled service won’t be available after cancellation. Make sure any important messages or details are saved before the cancellation is completed.
